NOTE: All waiting lists close from mid November until the start of February each year, excluding Baby intake.  

​

All new patients are required to submit "Wait List Intake forms".  

We are not able to give wait times estimations for our waiting lists. 

PLEASE NOTE: Preference for places will be given to local clients.  

​

Newborn / breastfed babies will submit Baby Intake forms, while there may be a wait time for some babies at times, priority is given based on severity and need. The more information you provide, the easier it will be to determine your priority status.

Please email us to request your Wait list Intake forms or Baby Intake forms.

During office closure times, please text the mobile for our Baby Intake forms.

Your wait list / baby intake form will be emailed to you, Please make sure you read the instructions carefully, you need all information required for you submission to be approved.

You will need to fill in the Patient Details form included in the email

• this form will need to be signed. 
   

If you have NDIS you will need to attach:

• a copy of your NDIS plan or information pag

• a signed copy of the service agreement included in the email 

• any other reports that may be relevant. 

​

The above paperwork can be attached into the Jotform Intake form, click on the link to fill in the paperwork, save to your computer and then attach your paperwork to the Jotform. 

If you are unable to attach your paperwork, you need to email this paperwork BEFORE you submit your jotform.

YEARLY APPOINTMENTS 

​LaLa Speech Pathology "regular" sessions are allocated at the beginning of each year by clients submitted preferences via Jotform.  

Jotform preferences are emailed out to all our existing clients before the end of each year with due dates for their return.  

All submissions received by the due date will be allocated in round 1 of allocations.  

All submissions received after the due date will be allocated in round 2, in order of submission.

SWAPPING YOUR SESSION TO TELEHEALTH 

​If you are unable to attend your appointment at the office for any reason, you can change your session to Telehealth. 

Contact reception or reply "TELE"  to your appointment reminder

We will email you a TEAMS link.

TEAMS 

You do not need to join TEAMS or set it up to accept the link.  

Just follow the the prompts from the emailed link.  

Make sure you enter your name when asked as this will add you to the waiting area. The therapist will add you in once they are ready.

Please make sure you have tested your microphone and camera before your appointment,

Children will get the most out of their sessions sitting in a good chair (not a swivel chair) at a table or desk in a quiet area away from TV, siblings etc. 

We suggest a parent is close by as the therapist is not able to see what your child is doing off screen or to assist if there are technical issues.  ​​​

CANCELLATION POLICY

 

ALL CANCELLATIONS need to be made with reception via phone, email or text.  You are not able to give notification to your therapist.  

​

Our cancellation fees are guided by NDIS guidelines.  

We recommend requesting to reschedule your session or using Telehealth for your session. 
Cancellation fees apply regardless of the reason for cancellation eg illness.  

Your child should not attend a session if unwell of any kind.  We recommend changing to Telehealth.

Cancellation fees will apply for all appointments cancelled as per the schedule below.  

 

CANCELLATION FEES ALL CLIENTS 

FAILURE TO ATTEND - 100%

Less than 2 clear working days – 100%

 

NOTES: Working days are Monday to Friday 9am to 3pm.   

9am appointments would require cancellation before 3pm the previous day to not be deemed “ON THE DAY” cancellation.

Group sessions will charged as per the Groups Agreement.   

​

Speech Pathology group sessions cancelled by us due to lockdowns will be charged for the first cancelled session per lockdown period, subsequent sessions in the same lockdown will not be charged regardless of the current Groups Agreement.
